A standardized PPE can reliably screen for cardiac, pulmonary, neurological, MSK, and psychological risk factors for incurring injuries while playing sports. Most risks can be identified with a thorough patient history and family history, a general physical exam, and targeted physical examination where indicated. Clinicians should have a low threshold to withhold participation clearance until further testing or referral to specialists can be completed. The study examines the utility of preparticipation sports physical evaluations (PPE) in identifying health risks in adolescent athletes. Using expert-guided recommendations and a review of current practices, 14% of patients require further evaluation and ultimately 98% of athletes are cleared for participation following PPE. Standardized forms improved sensitivity, though controversies persist regarding recommendation of routine ECG screenings. Adolescents often only see their provider once per year for the PPE, making it a critical opportunity for health promotion, identifying risks like sudden cardiac death or concussions, integrating preventive care, and establishing the medical home.
