The 2025 executive summary shows new developments in the AHA guidelines, integrating science, systems, and education with a unified Chain of Survival across the entire cardiac arrest continuum so that communities can prioritize which changes matter most for their patients, from lay rescuer response to prehospital care to the ED and beyond.
ROSC and rollout: The 2025 AHA playbook for cardiac arrest systems
Despite advances in science and insight, incidences of cardiac arrest remain high, both in and out of the hospital, with intact neurological survival only marginally improving. The 2025 AHA guidelines delve into a reorganized, system- and equity-based structure, reconciling overlapping content across approach to BLS, ALS, PALS, neonatal care, special circumstances, post-arrest care, systems of care, ethics, and education.
This is an executive summary of guideline development. Key results include: survival to discharge after EMS-treated adult OHCA remains 10.5% (favorable neurologic outcome 8.2%); adult IHCA survival 23.6% (79.2% good neurological outcome); 38% of recommendations rest on limited data (LOE C).

How will this change my practice?
Sometimes we function only within the walls of our EDs and forget what happens before or after patients arrive. For cardiac arrest, we must remove these blinders and realize that we are one link in a long chain of survival, where every step matters. We should be advocates of accessibility and best practices beyond our walls. As an emergency physician, I will continue to advocate for universal distribution and implementation of these standards across all aspects of my community, especially in the most marginalized populations.
