CRASH 3 – TXA for TBI
TXA appears to have a slight 28-day mortality benefit in traumatic brain injury (TBI) patients with a GCS ≥9, especially when given early.
EXTEND – Alteplase Up to 9 Hours Out?
Use of alteplase from 4.5 to 9 hours in patients with ischemic stroke but with salvageable brain on perfusion imaging was superior to placebo (aRR 1.44, 95%CI 1.01-2.06; NNT = 17). Symptomatic hemorrhage was more common in the alteplase group.
Early or Delayed Cardioversion for Atrial Fibrillation
In patients who presented with recent onset atrial fibrillation < 36h, delayed cardioversion at 48h compared to early cardioversion was non-inferior (91% vs 94%; -2.9 percentage points, 95%CI -8.2 to 2.2).
PE in Pregnancy – YEARS Algorithm
The Pregnancy-Adapted YEARS Algorithm safely and accurately ruled out pulmonary embolism (PE) for pregnant patients and reduced the rate of CT pulmonary angiography (CTPA) across all trimesters.
New Landmark PECARN Rule for Infant Fever
Infants ≤60 days are low risk for serious bacterial infection (SBI) if they have a negative urinalysis, ANC ≤4090/µL, and procalcitonin ≤1.71 ng/mL. NPV was 99.6%; negative likelihood ratio (NLR) 0.04. Keep in mind, this hasn’t yet been externally validated.

AIRWAYS-2 – SGA vs ETT for Out-of-Hospital Cardiac Arrest
There was no difference in good neurological recovery after non-traumatic out-of-hospital cardiac arrest at 30 days with use of a supraglottic airway compared to endotracheal tube.
EMS ETT or LT for OHCA? Answers in PART
For adults with out-of-hospital cardiac arrest, initial laryngeal tube insertion by emergency medical service providers was associated with improved 72-hour survival, return of spontaneous circulation, hospital survival, favorable neurologic outcome, and airway success compared to initial endotracheal tube insertion.
PAMPer – Prehospital Plasma for Hemorrhagic Shock Saves Lives
Prehospital administration of 2 units of thawed plasma in adult trauma patients at risk for hemorrhagic shock reduced mortality compared to standard care, NNT = 10.
